Primary synovial sarcoma on MRI – a case series and review of the literature

Abstract: PurposeSimple MRI features are mandatory to facilitate the diagnostics of synovial sarcomas, especially for radiologists outside multidisciplinary sarcoma centres. In this case-series and review, we investigate the main appearance of synovial sarcoma on MRI.Material and methodsFifteen histologically proven primary synovial sarcomas who underwent MRI at 2 different sarcoma centres were included in this case series. Patients and their primary synovial sarcomas were examined for age, localization, mean tumour siz… Show more

“…10 However, not all SSs exhibit the typical "triple sign". Sedaghat et al 11 performed MRI in 15 histologicallyconfirmed primary SS cases, revealing that 60% of the tumors displayed the triple sign. Liang et al 9 identified the triple sign in only 50% of their cases, while Ashikyan et al 12 noted its presence in 80% of SSs.…”

“…However, around one-third of larger lesions exhibit distinctive features of heterogeneity on T2weighted images, including the ''bowl of grapes'' appearance, indicating a multicystic mass with varying fluid intensities and "fluid-fluid levels", as well as the ''triple sign,'' signifying areas of hyperintensity, isointensity, and hypointensity relative to skeletal muscle [17]. The "bowl of grapes" appearance may arise due to the presence of hyperintense areas on fluid-sensitive sequences with interspersed T2 hypointense septa (Figure 6) [38][39][40]. This finding is not unique to synovial sarcoma and is also seen in undifferentiated pleomorphic sarcoma [41].…”
