Priority of lymph node dissection for advanced esophagogastric junction adenocarcinoma with the tumor center located below the esophagogastric junction

Abstract: To clarify the priority of lymph node dissection (LND) in advanced Siewert type II and III AEG, in which the center of the tumor is located below the esophagogastric junction (EGJ). Data in 395 patients with advanced Siewert type II or III AEG was analyzed retrospectively. The index of estimated benefit from LND (IEBLD) was used to evaluate the efficacy of LND for each nodal station. The mean number of dissected LNs did not differ significantly between patients with type II and III AEG,… Show more

“…Interestingly, the paracardial LN metastatic rate of vlESCC was still higher than that of Siewert I-II AEG in matched dataset 2 (36.1% vs. 19.7%). In other studies, the LNM rate of paracardial was 18.9% to 53.8% (15)(16)(17) for lower thoracic ESCC, and 26.1-32.9% for Siewert II AEG (11,18,20). The paracardial LNM rate in our unmatched series (35.3%) was consistent with the previous studies mentioned above.…”

“…The 5-year survival rate of patients with lymph node metastasis was calculated for each nodal station independently, irrespective of metastasis to other lymph node stations ( 20 ). The priority of nodal dissection was assessed by the value of the index at each station that had an IEBLD above 3.0 ( 21 ).…”

“…[19] Radical surgery has been proposed as the effective treatment for AEG, including complete resection of lesions and LND. [20,21] However, Siewert II/III AEG is characterized by specific anatomical location and complex lymphatic drainage direction. Therefore, the correct evaluation of the LNM can determine the extent of optimal LND.…”
