Problems Encountered in Long‐term Treatment with Anticoagulants

Abstract: The course of long-term anticoagulant therapy in 114 out-patients has been evaluated over a three-month period. The evaluation was based on the registration of information from the clinical records and from two personal interviews with each patient. The patients had attended the Anticoagulation Clinic for 141 weeks on the average. The prothrombin complex activity (PP%) level was significantly lower in patients with bleeding episodes. “…Treatment with oral anticoagulants under international normalized ratio (INR) self-management can reduce thromboembolism to 1.5% per patient year (ppy) [1]. Reported bleeding rates vary with the quality of anticoagulation, ranging from 4.2 to 15.4% ppy [2][3][4][5][6].…”

“…These complications occur most frequently during the first 6 months after surgery. Later on, the risk decreases and remains constant for years [4][5][6]. Anticoagulation-related risk is higher when long-term INR variance is higher.…”

“…Bleeding complications and thromboembolic episodes are the most common problems in anticoagulant therapy. The difficulties involved in maintaining therapeutic levels have to do with the question of dosage and the now well known interactions between the anticoagulant drug and other drugs as well as alimentary products (3, 4, 6,13). Reports on liver damage secondary to anticoagulant therapy are more scarce and none have so far dealt with warfarin.…”
