Procalcitonin and C-reactive protein do not discriminate between febrile reaction to anti-T-lymphocyte antibodies and Gram-negative sepsis

Abstract: Summary:Treatment with antibodies against T-lymphocytes usually triggers a febrile response potentially mimicking or masking infection. Procalcitonin (PCT) is considered a sensitive and specific marker of systemic bacterial and fungal infection. It was the aim of this study to investigate the characteristics of PCT and C-reactive protein (CRP) during treatment with polyclonal or monoclonal anti-Tcell antibodies, in order to examine the ability of these parameters to distinguish between systemic bacterial infec… Show more

“…Furthermore, PCT elevation similar to Gram-negative sepsis was recently observed after T-cell antibody infusion in adult renal transplant patients [20] and in adults and children receiving T-cell antibodies mostly as conditioning treatment before allogeneic stem cell transplantation [7]. Blijlevens et al first reported single cases of GvHD associated with moderately increased PCT serum levels [2].…”

“…Clinical manifestations of disease often appear too late to allow for meaningful intervention. Commonly used parameters, such as measurement of C-reactive protein (CRP), leukocyte subsets and concentrations of pro-calcitonin are nonspecific and therefore unreliable [1,2]. In addition, obese women demonstrate higher levels of CRP and tumor necrosis factor (TNF)-alpha when compared to patients with lower body mass index (BMI) [3].…”

“…It is a mixture of antibodies against T cells which is used in the prevention and treatment of acute rejection in organ transplantation, and therapy of aplastic anemia. Side effects associated with ATG administration are common and include inflammatory reactions with fever and chills, impaired renal function, thrombocytopenia, leukopenia, skin rashes, circulatory instability, respiratory insufficiency and release of inflammation markers (Brodska et al 2009;Dornbusch et al 2003;Pihusch et al 2002). These reactions vary in intensity among different patients and in some cases may resemble sepsis.…”
