Prognostic factors for return to work after a first compensated episode of back pain.

Abstract: Objectives-To determine which factors measured at baseline and during the course of treatment influence time to return to work after a first compensated episode ofback pain. Methods-The design is a treatment inception cohort including 305 compensated workers out of 402 eligible ones presenting at two rehabilitation centres for conventional treatment. Crude and adjusted rate ratios (RRs) along with 95% confidence intervals (95% CIs) were estimated with the Cox's proportional hazards regression.

“…In studies in a primary care setting the rate of return to work was much higher and functional disability at inclusion was lower (16,(26)(27)(28). Patients in studies in a specialized clinic or a rehabilitation center represented more chronic cases when compared with patients in this study (29)(30)(31). However, compared with other studies in an occupational health setting, our study had more serious cases (8-10).…”

“…This explanation is backed up by the comments of some of the participants to the focus groups. 1 In several previous studies, increasing age was associated with an adverse evolution of back pain [15,31,34,44,55,74,79]. Others have however concluded to an inverse (protective) effect of age [1,18].…”

“…This may reflect that women in high level jobs have to joggle both with the stress of the job in addition to the stresses and hassles of family life. Current evidences about this association are contradictory [39,44].…”

“…For a prediction of the chances of return to work, such an inception cohort would consist of workers who are sick-listed because of back pain for less than 4 weeks. It can be concluded that chances of return to work are diminished if workers are older or if they have a higher level of initial disability, a specific diagnosis, or radiating pain (18,(38)(39)(40)(41). From the same studies it can be inferred that heavy physical work does not predict time to return to work.…”
