Prognostic Impact and Post-operative Evaluation of Volumetric Measurement of the Cerebellopontine Cistern in Trigeminal Neuralgia Using 3 Tesla Magnetic Resonance Imaging

Abstract: The aim of this study was to evaluate the importance of pre- and post-operative volumetric measurement of the cerebellopontine angle (CPA) using 3 Tesla (3T) magnetic resonance imaging (MRI). Between April 2012 and December 2015, a total of 87 consecutive patients underwent microvascular decompression (MVD) for trigeminal neuralgia (TN), of whom 51 with primary TN caused by arterial compression were enrolled in this study. Bilateral CPA cistern volume was evaluated using 3T MRI before and after surgery; the Ci… Show more

“…MR could predict prognosis of primary TN. Obata et alfound that smaller preoperative cistern deviation index was positively correlated to the recurrence of TN after microvascular decompression (MVD) [ 14 ]. What’s more, Andrei Brînzeu et al [ 28 ] and Patrick W Hitchon et al [ 30 ] found degree of NVC on MR could accurately predict the outcomes of patients with TN who underwent MVD.…”

“…The reformatted images were magnified to facilitate the measurement of the length and volume of the target. Morphometric length and volume parameters were determined and measured as previously described [ 15 , 22 , 23 ]. The length measurements were calculated by manually drawing on axial images with a section thickness of 0.5 mm, and the volume measurements were calculated automatically with the 3D reconstruction software.…”

“…Hung et al suggested that diffusion tensor imaging (DTI) could assist the diagnosis of TN 16 . Researches revealed positive correlations between nerve atrophy, small trigeminal pontine angle, small CPA cistern cross-sectional area, short trigeminal nerve, thin trigeminal nerve and TN 6,11,12,19,29,30,40 .Wei et al reported that three-dimensional time-of-ight (3D-TOF) MR angiography imaging showed a sensitivity rate (SR) of 74.0% in the evaluation of NVC, the SR could reach to 88.4% as it was accompanied by 3D-CISS 38 . Boecher et al reported an 88.9% of positive rate (PR) and 88.5% of SR of NVC from high-de nition MR angiography 3 .…”

“…(Table 6) Many factors in uence the outcome of TN after MVD, MR might predict prognosis of primary TN as well. Obata et al found that smaller preoperative cistern deviation index was positively correlated to the recurrence of TN after MVD 29 . What's more, Brinzeu et al 4 and Hitchon et al 13 found that the degree of NVC on MR could accurately predict the outcomes of TN after MVD.…”
