Progressive Atrioventricular Block after Total Correction of Fallot’s Tetralogy, Documented by Repeat Electrophysiological Studies

Abstract: A case of progressive trifascicular block late after correction of Fallot’s tetralogy is reported. Progressive damage to the His bundle and/or its branches over 18 months after surgery was suggested by sequential ECGs and confirmed by repeat electrophysiological studies done 3 and 18 months postoperatively: the H-V interval increased from 55 to 80 ms over this period. With atrial pacing at increasing rates, supra-His 2:1 atrioventricular (AV) block occurred at a rate of 158/min at the first study; at the secon… Show more

“…It has been shown that the incidence of delayed complete atrioventricular block is 0.3% to 0.7% in patients undergoing cardiac surgery including the closure of a ventricular septal defect 8 . As progressive atrioventricular block late after Fallot correction has been documented 9 and as late sudden death is associated with late complete atrioventricular block in Fallot patients, 10 our results confirm the necessity of repeated electrocardiographic follow‐ups of adults with operated tetralogy of Fallot.…”

“…We reviewed the literature about the late CHB in PubMed, Embase, Medline using the following keywords: Teratology of Fallot, Complete heart block, Heart block, AV block, AV dissociation and we ended up with 19 cases 10 13–15 17 19–22. Most of the cases with predictable CHB, 70%, had transient CHB.…”
