Prolonged Carotid Sinus Reflex Is a Risk Factor for Contrast-Induced Nephropathy following Carotid Artery Stenting

Abstract: BACKGROUND AND PURPOSE:Although many studies have demonstrated that CIN is associated with in-hospital and long-term mortality, the incidence of CIN following CAS is unclear. We investigated the incidence of CIN, defined as an increase from a baseline creatinine value of at least 0.5 mg/dL or 25% within 72 hours of contrast administration, and we also examined renal function in the late phase after CAS.

“…This rate is similar to previous reports on neurointerventions [9, 10]. CIN occurred in 7/80 (8.8%) patients after carotid-artery stenting [9], and in 16/192 (8.3%) patients after coil embolization with an aneurysmal subarachnoid hemorrhage [10]. Recently, CIN after mechanical thrombectomy occurred in 11/330 (3.3%) patients [11], a lower incidence than in our study, probably due to the younger study population (63.9 ± 15.8 vs. 74.5 ± 11.5 years).…”

“…[1][2][3] A potentially worrying complication (that is increasingly being reported after CAS) is the development of contrast-induced acute kidney injury (CI-AKI). [4][5][6][7][8][9] This review discusses CI-AKI as a complication of CAS.…”

“…This is possibly because all of our patients had bilateral high-grade CS, while patients in other investigations just had either unilateral CS or bilateral moderate- or middle-grade CS; therefore, the extent of stenosis in the present investigation was more severe than that in other investigations. As HD often lasts a long time and can result in renal failure, it usually requires careful observation and management [52], although usually the SBCAS-related HD is not very severe and commonly does not increase perioperative risk. Because we had no cases of HD developing secondary to cerebral ischemia or myocardial infarction, we believe that cautious perioperative management helps to effectively reduce the incidence of HD.…”
