Prolonged Pulseless Electrical Activity Cardiac Arrest After Intranasal Injection of Lidocaine With Epinephrine: A Case Report

Abstract: Local anesthetic toxicity is a rare but serious complication of local anesthetic administration. Although lidocaine has a safety profile superior to other amide local anesthetics, we report a case of cardiac arrest after intranasal injection of lidocaine. The case involves a 22-year-old healthy woman who experienced pulseless electrical activity shortly after a submucosal injection of 2.2 mg/kg of lidocaine with epinephrine. Resuscitative efforts were unsuccessful until a bolus of intralipid was given. This ca… Show more

“…In Rosenblatt et al (32) used 20% lipid emulsion infusion and successfully resuscitated a patient whose heart was arrested for a long time as a result of bupivacaine toxicity. Thus, the effectiveness of lipid emulsions has been verified in clinical cases; many reports have documented successful recovery from LAST with lipid emulsions (33)(34)(35). Lipid emulsion therapy has also achieved good results as a treatment for LAST in newborns and children, not just in adults (36,37).…”

“…93 Local anesthetic systemic toxicity (LAST) can cause devastating arrhythmia and POCA. [94][95][96][97][98][99][100][101][102][103][104][105][106] Bupivacaine can induce severe myocardial depression 107 and cardiac toxicity, mimicking an acute non-ST segment elevation myocardial infarction. 108 One case described a 76-year-old woman undergoing outpatient TEE and atrial fibrillation cardioversion.…”

“…Between December 2017 and May 2020, there were a further 36 cases of LAST reported in 34 peer‐reviewed published articles [6–39]. These data indicated 30% of patients were older than 65 y, and 6% were younger than 5 y. Sixty‐one percent of cases occurred in the hospital; 17% in outpatient surgery centres; and 14% in outpatient clinics (e.g.…”
