Prophylactic Mesh Placement During Formation of an End-colostomy Reduces the Rate of Parastomal Hernia

Abstract: Prophylactic augmentation of the abdominal wall with a retromuscular lightweight polypropylene mesh at the ostomy site significantly reduces the incidence of PSH without a significant difference in morbidity, mortality, quality of life, or cost-effectiveness.

“…The rising prevalence of PSH is due to improvements in survival after rectal cancer surgery and an increase in the rate of obesity . From a pathophysiological point of view, elevated intra‐abdominal pressure increases tension along the abdominal wall (Laplace's law), and therefore the diameter of the stoma gradually grows .…”

“…The rising prevalence of PSH is due to improvements in survival after rectal cancer surgery and an increase in the rate of obesity [8,9]. From a pathophysiological point of view, elevated intra-abdominal pressure increases tension along the abdominal wall (Laplace's law), and therefore the diameter of the stoma gradually grows [10]. PSH can be repaired by primary suture of the defect or by parastomal mesh (PSM) (keyhole techniques, Sugarbaker-type repairs or sandwich procedures) [11].…”
