Prophylactic Platelet Transfusions in Children with Acute Leukemia: A Dose Response Study

Abstract: A study of the value of small doses of platelets given prophylactically in thrombocytopenic patients was carried out. Transfusions of two dose levels of platelets were compared: 0.03 units/1b (low dose) and 0.06 units/lb of body weight (high dose). The incidence of bleeding in nontransfused patients seen here prior to this study was also determined. The difference in incidences of bleeding among patients transfused with low or high dose was not significant. The differences between the transfused and nontransfu… Show more

“…Minor bleeding episodes in skin, mucous membranes, microscopic hematuria, guaiac positive stools, and epistaxis were found in approximately 50% of individuals with platelet counts between 0 and 40,000 / p,l but there was no correlation between frequency of bleeding and absolute counts within that range (64). However, serious bleeding (gross bleeding from GI or GU tract) occurred in 26% of the 62 patients with platelet counts between 0 and 10,000 / p,l, 10% between 10,000 and 20,000 / p,l and 4-5% between 20,000 and 40,000 / p,l.…”

“…5 Roy et al randomly assigned 62 patients to receive 1 of 2 doses, based on units per body weight, when the platelet count was Յ 25 000/L. 1 Both doses were equally efficacious in reducing bleeding incidence, but the small sample size precluded definitive dosing conclusions.…”

“…Disadvantages using profi lactic PC therapy include increased number of donor exposures and possibly also enhanced risk of alloimmunzation and refractorines [35,36,10,11]. Th e advantages of profi lactic therapy are still not universaly accepted.…”
