Propranolol stereoisomer plasma concentrations and portal haemodynamic response in patients with liver cirrhosis

Abstract: Our data demonstrate a stereoselective metabolism of propranolol enantiomers in liver cirrhosis. However, following oral propranolol administration, stereoisomer plasma concentrations do not predict the portal haemodynamic effect.

“…up to 15 ng/mL after oral application, clenbuterol up to 30 ng/mL, toremifene >1000 ng/mL, trenbolone 50 ng/mL (in horse plasma), exemestane up to 50 ng/mL, salbutamol 10 ng/mL after oral application, formoterol up to 20 ng/mL after inhalative administration (i.v. ), resveratrol 40 ng/mL and GW1516 in a range of 40 to 700 ng/mL 29–44. Plasma concentrations of over 5 µg/mL for the selected peptides were reported for growth hormone release peptide‐2 (GHRP‐2) after intravenous administration of 3 mg/kg (in rats) 45.…”

“…When the sample point was adjusted to the center of the portal vein, the portal venous velocity was recorded during a quiet suspended expiration and was averaged over a few seconds. The PVF was determined by the formula: crosssectional area × mean velocity × 60 (20). The coefficient of variation for the PVF measurements with this method in our center is 3% (21).…”

“…In patients with liver disease the concentration of the S‐propranolol is greatly elevated. Stereoisomer concentrations, however, do not appear to predict the haemodynamic response to propranolol, and there is no difference in the relative concentrations of both stereoisomers in propranolol responders and nonresponders 24 …”
