Study design Retrospective population-based cohort study. Objectives To determine the incidence and mortality of spinal cord injuries (SCI) in the Piedmont Region of Northwestern Italy. Setting Publicly-funded SCI rehabilitation centres in the Piedmont Region. Methods Administrative databases were used to identify individuals at their first admission to a SCI rehabilitation centre from January 1st, 2008 to December 31st, 2020. Cases were stratified by age and aetiology (traumatic SCI, TSCI; non-traumatic SCI, NTSCI). Age- and aetiology-specific incidence rate and person-year mortality rates were calculated for each year. Case lethality was reported as deaths among prevalent cases for each year. Results A total of 892 cases were identified (56.4% TSCI). The average annual crude incidence rate was 17.9 per million population, decreasing from 26.0 in 2008 to 10.8 in 2020. Young adults and the elderly represented the majority of TSCI and NTSCI cases, respectively. Of the 235 individuals who died during the study period, 58.3% had NTSCI. The mortality rate per 1000 person-years decreased from 16.3 in 2009 to 8.5 in 2020, while case lethality more than tripled (from 17.2 in 2009 to 57.1 in 2020). Conclusions We identified a decreasing trend in SCI incidence and mortality rates, with an increased case lethality over the study period, especially in NTSCI. Given these changes in the epidemiology of SCI, community services offered after rehabilitation should be strengthened to enhance their effectiveness and contribute to increased survival in this population.

A lesão medular (LM) repercute altamente no nível de incapacidade física. Pessoas com LM que utilizam dispositivo auxiliar de mobilidade tendem a desenvolver maior independência funcional. O objetivo foi avaliar a correlação entre independência funcional em atividades de vida diária e a satisfação com o uso de tecnologia assistiva (cadeira de rodas) em pessoas com lesão medular. Trata-se de um estudo transversal analítico com abordagem quantitativa. A amostra foi de 146 participantes com lesão medular traumática com paraplegia e tetraplegia. Para coleta de dados foram utilizados um questionário sociodemográfico, e as escalas Spinal Cord Independence Measure-Self Reported Version (brSCIM-SR) e o Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST.2.0) que permitiram a investigação. A média de idade foi 37,8 anos, com maioria do sexo masculino (82,9%), solteiros (52,7%), escolaridade média (53,4%), acidente de trânsito (50%), arma de fogo (22,6%). Houve predomínio de lesão completa (58,9%) e paraplegia(65,1%). Na brSCIM-SR a média geral foi de 55,3 (±15,8) no domínio do autocuidado de 14,6 (±5,3), respiração/esfíncter 26,4 (±7,4) e mobilidade de 14,3 (±5,5). Na satisfação com a tecnologia assistiva a pontuação total do QUEST 2.0 foi de 3,64 (±0,77), satisfação com o recurso 3,86 (±0,74) e a prestação de serviços 3,64(±0,77). Identificou-se bom desempenho na média da independência funcional. Além disso, altos níveis de satisfação com a tecnologia assistiva, de forma específica mais com o recurso do que com a prestação de serviços. Não foi encontrada correlação entre a independência funcional e a satisfação do uso de cadeira de rodas.

BACKGROUND: Epidural spinal cord stimulation (ESCS) emerged as a technology for eliciting motor function in the 1990’s and was subsequently employed therapeutically in the spinal cord injured (SCI) population. Despite a considerable number of ESCS studies, a comprehensive systematic review of ESCS remains unpublished. OBJECTIVE: The current review of the existing literature evaluated the efficacy of ESCS for improving motor function in individuals with SCI. METHODS: A search for ESCS studies was performed using the following databases: Medline (Ovid), Web of Science and Embase. Furthermore, to maximize results, an inverse manual search of references cited by identified articles was also performed. Studies published between January 1995 and June 2020 were included. The search was constructed around the following key terms: Spinal cord stimulation, SCI and motor response generation. RESULTS: A total of 3435 articles were initially screened, of which 18 met the inclusion criteria. The total sample comprised of 24 participants with SCI. All studies reported some measure of improvement in motor activity with ESCS, with 17 reporting altered EMG responses. Functional improvements were reported in stepping (n = 11) or muscle force (n = 4). Only 5 studies assessed ASIA scale pre- and post-intervention, documenting improved classification in 4 of 11 participants. Appraisal using the modified Downs and Black quality checklist determined that reviewed studies were of poor quality. Due to heterogeneity of outcome measures utilized in studies reviewed, a meta-analysis of data was not possible. CONCLUSION: While the basic science is encouraging, the therapeutic efficacy of ESCS remains inconclusive.
