Prospective, randomized trial examining the role of gentamycin-containing collagen sponge in the reduction of postoperative morbidity in rectal cancer patients: early results and surprising outcome at 3-year follow-up

Abstract: The use of the gentamycin collagen sponge after excision of rectal cancer is safe and reduces the rate of early postoperative complications. The reasons for the lower rate of distant metastasis in the GRM(+) group are not clear, but the patients enjoy significant survival benefits.

“…Local application of gentamicin-collagen has been used both to prevent and treat surgical wound infection 12 -14 . This treatment has been demonstrated to improve perineal wound healing after abdominoperineal excision of rectal cancer 15 and to reduce postoperative complications after rectal cancer surgery 16 . The primary aim of the present study was to evaluate whether local antibiotic treatment with gentamicin-collagen improves healing after endoanal advancement flap repair for anal fistula.…”

“…Several studies suggest that the sponge may be effective in the prevention and treatment of infections after general surgery. 6,13,14 In a single-center, randomized trial, patients who underwent colorectal surgery and received a sponge had a 70% decrease in surgical-site infection, as compared with those who did not receive a sponge. 6 The current phase 3 trial was designed to confirm these promising data and support regulatory approval in the United States.…”
