Background. Thoracic outlet syndrome may be due to various reasons and be classified as neurogenic, arterial, and venous thoracic outlet syndrome. The surgical treatment of neurogenic thoracic outlet syndrome can be performed using either a trans-axillary or a supraclavicular approach. The aim of this study is to report on such patients operated in a single institution by a single thoracic surgeon using trans-axillary approach. Methods. The patient files were screened for patients operated due to neurogenic thoracic outlet syndrome related symptoms between September 1, 2002 and April 1, 2015 in the Department of Thoracic Surgery of Bursa Yuksek Ihtisas Training and Research Hospital. Results. There were 22 female and five male patients with an average age of 31.9±11.7 years. The most common symptom was pain in the affected limb and shoulder (n=29). Of the diagnostic maneuvers abduction external rotation (Roos) test was the most frequently found positive test (n=27). Nerve conduction velocity studies revealed an average ulnar nerve conduction velocity of 62.7±6.6 m/s. Limited pneumothorax uneventfully resolved in 2 days was seen following 12 operations. Conclusion. We conclude that trans-axillary approach may provide good exposure with favorable opportunity to excise the 1st and the cervical rib, and neurolysis with a low rate of complication.

Purpose: Thoracic outlet syndrome is one of the most controversial topics in clinical medicine due to its difficult assessment and management. The appropriate treatment depends on accurate and comprehensive assessment. This study aimed to review the current assessments of these patients. Methods:This study was conducted by review of the articles published between 1990 and 2014. Search was conducted by keywords such as assessment, evaluation, outcome measure, tool, thoracic outlet syndrome, non-surgical treatments, conservative, and rehabilitation. In this regard, internal and external databases, including PubMed, OVID, ProQuest, Web of science, Elsevier, OT seeker, SID, Magiran, Iran Medex, Medlib, and Google scholar were used. Results:The results showed that the most common assessments for these patients in a clinical and research setting included a thorough review of history and subjective evaluation. No specific tools were found for patients with thoracic outlet syndrome but 5 generic outcome measures were suggested to measure the outcome of interventions in these patients. Conclusion:At present, comprehensive evaluation of patients with thoracic outlet syndrome needs different assessments and using the generic questionnaire that is designed for people with orthopedic conditions. Therefore, a comprehensive assessment tool with a holistic view and specific for patients with thoracic outlet syndrome is necessary.

Thoracic outlet syndrome (TOS) is one of the common neurovascular disorders of the upper extremities, and the compression or traction of the brachial plexus is the main pathology. We hypothesized that baseball players are more likely to be affected by TOS compared with other overhead-throwing athletes because of strenuous use of their throwing arm. The purpose of this study was to investigate the prevalence and clinical characteristics of TOS in high school baseball players. One thousand two hundred eighty-eight high school baseball players were included in this study. The prevalence of symptomatic TOS and its association to disorders of the upper extremities were investigated. The prevalence of symptomatic TOS was 32.8%. Age-and position-adjusted logistic regression analysis revealed that subjects with symptomatic TOS were at significantly higher risk of shoulder and/or elbow pain (odds ratio [OR]: 1.96, 95% confidence interval [CI]: 1.50 -2.58) and a higher recurrence rate of shoulder and/or elbow pain during the previous season (OR: 2.38, 95% CI: 1.87 -3.04). As for physical findings, subjects with symptomatic TOS were also at significantly higher risk of MUCL tenderness (OR: 2.53, 95% CI: 1.83 -3.50), positive milking maneuver, (OR: 2.63, 95% CI: 1.78 -3.91), positive subacromial impingement sign (OR: 1.92, 95% CI: 1.20 -3.07), and positive posterior impingement sign (OR: 2.34, 95% CI: 1.67 -3.28) compared to the subjects without symptomatic TOS. It is necessary to recognize that TOS is not a rare pathology in overhead-throwing athletes, especially baseball players, and that players with symptomatic TOS are at significantly higher risk of shoulder/elbow pain.

Abstract: DASH questionnaire is a valid and objective test for evaluating the functional state after TOS surgery. Venous TOS is clinically less incapacitating than neurogenic. Surgically decompression of thoracic outlet leads to significant benefit in patients with neurogenic TOS.
