BackgroundEccentric instruments have been proposed as more effective and less time‐consuming for endodontic instrumentation.AimTo compare biomechanical outcomes of different instrumentation systems and time undertaken for instrumentation in resin prototypes.DesignSixty standardized prototypes of mandibular second primary molars were instrumented according to the following systems: K‐files, ProTaper Next (PTN), XP‐endo Shaper (XPS), XP‐endo Finisher (XPF), XP–Clean (XPC), and Sequence Baby File (SBF; n = 10/each). Irrigation was performed with saline with simultaneous aspiration, and time spent was recorded. The prototypes were micro‐CT‐scanned before and after the instrumentation, and image sets were reconstructed and registered. Non‐instrumented areas, accumulated debris, removed root material volume, and canal transportation were quantified. Data were analyzed through ANOVA, the Kruskal–Wallis test, and the Wilcoxon signed‐rank test (α = 5%).ResultsK‐files and SBF resulted in more instrumentation time (p < .05). SBF, XPC and PTN removed less root dentine (p < .05), but PTN left more untouched areas (p < .05). Accumulated debris were lower for XPC and SBF (p < .05). Canal transportation was similar among the groups.ConclusionRotary systems reduced instrumentation time, whereas SBF and XPC resulted in more conservative instrumentation, with less debris accumulation and non‐instrumented areas. A dedicated paediatric endodontic system (SBF) outperformed eccentric instruments in terms of effectiveness.

Background Managing dental caries in primary teeth with pulp involvement is a significant challenge. Clinical guidelines offer recommendations for effective management. Aim To identify and analyze policies, guidelines, and recommendations for treating primary teeth with pulp-involved carious lesions, highlighting existing research gaps and setting the foundation for future research. Methods A comprehensive search was conducted across databases (PubMed, Scopus, Embase, GIN, and LILACS) and grey literature sources (Trip and ProQuest) to identify guidelines, consensus, policy, and position statements on primary teeth pulp therapy and extraction thresholds. Two independent reviewers screened the abstracts and titles, followed by full-text screening. Results After removing duplication, of the 1098 records, 14 were selected for analysis. This review examined various treatments for deep caries lesions in primary teeth, including indirect/direct pulp capping, pulpotomy, pulpectomy, lesion sterilization/tissue restoration, and extraction. Time search was restricted to documents published from 30th January 2008 to 30th January 2024, offering insights into evolving clinical practices. Conclusion Treatment for carious lesions in primary teeth involving the pulp depends on clinical indications and may involve minimally invasive techniques. Recommended options are indirect pulp capping, pulpotomy, and pulpectomy, while direct capping and tooth removal are discouraged. Further research is needed to address gaps, improve guideline development, and enhance consistency of recommendations.

OBJETIVO: Analisar, através de revisão da literatura, os tipos de traumatismo dentário e as consequências diante de uma intervenção tardia na primeira infância. METODOLOGIA: Esta pesquisa é uma revisão de literatura, e seguiu as buscas nas bases de dados MEDLINE via PubMed, LILACS e SciELO. Foram utilizados termos indexados do Descritores em Ciências da Saúde (DeCS/MeSH), sendo as palavras-chaves utilizadas: Traumatismos dentários, Odontopediatria e Saúde Bucal. O período para realização da pesquisa ocorreu de julho a outubro de 2022. Foram incluídos estudos observacionais, caso clínico e revisão de literatura, com texto completo disponível, que possuam relevância para responder à questão norteadora da presente pesquisa, e que tenham sido publicados no período de 2012 a 2022. Foram excluídos os estudos de acesso restrito, teses, dissertações, trabalhos de conclusão de curso e os que estiverem escritos em idioma diferente do português, como também foram excluídos os estudos que não abordassem essa temática. RESULTADOS: Após aplicação dos critérios de inclusão e exclusão, permaneceram, para leitura completa, 08 artigos, que destacaram o atendimento de urgência como fundamental, para que se tenha uma maior chance na reabilitação bucal do paciente, sendo importante o conhecimento dos pais e cuidadores. CONCLUSÃO: Pode-se concluir que é de suma importância o diagnóstico e tratamento em tempo hábil, para evitar piores prognósticos, nas diversas situações causadas pelo traumatismo dentário, sendo essencial que o cirurgião-dentista esteja preparado para realizar o acompanhamento clínico e radiográfico dos casos.
