BACKGROUND: Undocumented migrant families face many challenges in bringing their children for vaccination. The recent outbreak of poliomyelitis in Sabah among undocumented migrant children highlights the gap in vaccination coverage among these populations. AIM: This study aimed to determine the factors that influence undocumented migrants in Sabah to vaccinate their children. METHODS: This is a cross-sectional study using a questionnaire developed based on the findings from a qualitative study. This study was conducted in 15 districts of Sabah among the mothers of undocumented migrant children. Multiple Logistic Regression was done to find out the significant factors that influence undocumented migrants to vaccinate their children. RESULTS: A total of 942 responses were collected during the study. About 78.7% of the respondents attended health facilities to vaccinate their children. Only the legal status of the undocumented migrant (adjusted odds ratio [aOR] = 0.15 [95% CI: 0.52–0.43]) remains a significant militating factor for undocumented migrants to vaccinate their children. Two factors, parental trust in health-care providers and vaccines (aOR = 18.24 [95% CI: 8.42–39.51]) and good support system (aOR = 2.65 [95% CI: 1.77–3.97]), remain significant motivating factors that influence undocumented migrants to vaccinate their children. Those who had visited the health facilities for an antenatal check-up (aOR = 25.93 [95% CI: 17.07–39.39]) and delivery (aOR = 93.63 [95% CI: 34.77–257.81]), with income of more than RM 1000 (aOR = 6.09 [95% CI: 3.66–10.12]) have a higher prevalence to bring their children for vaccination. CONCLUSION: In the best interest of public health and to prevent the further re-emergence of vaccine-preventable diseases in Sabah, it is important to address these factors to improve vaccine uptake among undocumented migrant children.

Background In 2011 Norway granted undocumented women the right to antenatal care and to give birth at a hospital but did not include them in the general practitioner and reimbursement schemes. As a response to limited access to health care, Non-Governmental Organizations (NGO) have been running health clinics for undocumented migrants in Norway’s two largest cities. To further facilitate universal health coverage, there is a need to investigate how pregnant undocumented women use NGO clinics and how this affects their maternal health. We therefore investigated the care received, occurrence of pregnancy-related complications and pregnancy outcomes in women receiving antenatal care at these clinics. Methods In this historic cohort study we included pregnant women aged 18–49 attending urban NGO clinics from 2009 to 2020 and retrieved their medical records from referral hospitals. We compared women based on region of origin using log-binominal regression to estimate relative risk of adverse pregnancy outcomes. Results We identified 582 pregnancies in 500 women during the study period. About half (46.5%) the women sought antenatal care after gestational week 12, and 25.7% after week 22. The women had median 1 (IQR 1–3) antenatal visit at the NGO clinics, which referred 77.7% of the women to public health care. A total of 28.4% of women were referred for induced abortion. In 205 retrieved deliveries in medical records, there was a 45.9% risk for any adverse pregnancy outcome. The risk of stillbirth was 1.0%, preterm birth 10.3%, and emergency caesarean section 19.3%. Conclusion Pregnant undocumented women who use NGO clinics receive substandard antenatal care and have a high risk of adverse pregnancy outcomes despite low occurrence of comorbidities. To achieve universal health coverage, increased attention should be given to the structural vulnerabilities of undocumented women and to ensure that adequate antenatal care is accessible for them.

Delivering healthcare to undocumented migrants presents a complex challenge for healthcare providers. Integrating advocacy efforts into their daily practices can be ambiguous in practical terms, stemming from the intricate task of addressing the health needs of this population while simultaneously advocating for their health rights within the constraints imposed on them. This study seeks to consolidate findings from literature regarding the advocacy approaches employed by healthcare providers and the correlated ethical challenges. We conducted a scoping review of qualitative literature by systematically searching four databases—PubMed/Medline, Embase, Cinahl, and Cochrane Library. For developing our search strategy, we employed the PICO (Population, Intervention, Comparison, Outcome) scheme. Our analysis followed the qualitative content analysis approach proposed by Graneheim and Lundman. 30 studies were included, revealing a cumulative total of 915 healthcare providers who were interviewed. A total of 30 themes emerged comprising 14 advocacy approaches and 16 ethical challenges. Healthcare providers made a deliberate choice to engage in advocacy, responding to injustices experienced by undocumented migrants. The spectrum of advocacy initiatives varied, encompassing voluntary participation in healthcare provision, empathetic understanding, and healthcare-focused strategies. We also identified numerous correlated ethical challenges, necessitating healthcare providers to strike a balance between their eagerness to assist and their professional competence, respect the autonomy of undocumented migrants, and establish trust with them. These findings not only offer practical guidance for healthcare providers to enhance accessibility to healthcare services for undocumented migrant patients but also foster awareness of the ethical challenges that may arise in their advocacy roles.
