Pulmonary resection for lung cancer with malignant pleural disease first detected at thoracotomy

Abstract: The prognosis of patients with surgically detected MPD, who underwent resection, was better than that of M1b patients. MPE patients with N0-1 disease may be candidates for resection.

“…Third, the N factor and EGFR mutation status were not significant prognostic factors in the current study. However, previous studies found that the N factor and EGFR mutation are significant prognostic factors (2,4,10). Additional patient data may be required to arrive at a definitive conclusion regarding whether the N factor and EGFR mutation status may serve as prognostic factors.…”

“…In patients with intrathoracic metastasis, when malignant pleural disease (MPD) is first detected at thoracotomy, it is controversial whether the primary lesion should be resected (2). However, primary lesion resection had survival benefits for certain patients with unexpected intraoperatively p r o v e n M P D o r M P D p a t i e n t s w i t h o u t p l e u r a l effusion (3).…”

“…Among them, whether to undergo surgical resection of the primary tumor or not is still controversial. A few studies have been published while their findings were inconclusive (8)(9)(10)(11)(12)). Yet no randomized controlled trial has been performed to assess the role of primary tumor resection in NSCLC patients with MPD first detected during surgery.…”

“…However, recent publications have reported 5-year survival rates of .6% in patients with malignant pleural disease discovered at thoracotomy [169,170]. Five-year survival seems to be better in patients with N0-N1 disease, in those with pleural nodules and those undergoing lobectomy [169,170].…”
