Pure NOTES sigmoid resection in an animal survival model

Abstract: The presented study shows that pure NOTES resection and anastomosis of the large bowel are feasible. Intraluminal organ manipulation provided excellent organ exposition and rendered one additional access site unnecessary. Transgastric preparation was shown to be safe and effective.

“…Publication bias likely impacts reported adverse event rates. Expanding to EUS-CPN performed for any indication this list includes infarction of spleen, pancreas and gastric antrum,[ 10 ] gastric ulceration,[ 28 ] fatalities from bowel infarction,[ 12 13 29 ] retroperitoneal abscess or bleeding,[ 9 30 ] permanent paralysis/paraplegia,[ 31 32 33 ] brain abscess (but unclear if CPB or CPN),[ 11 ] pulmonary embolus,[ 34 ] and bilateral diaphragm paralysis. [ 35 ] Paraplegia is felt to occur by inadvertent injection of ethanol into the artery of Adamkiewicz causing vessel trauma, thrombosis or vasospasm with anterior spinal artery infarction of the spinal cord.…”

“…Pure NOTES resection and anastomosis of the large bowel were feasible, and the colorectal anastomosis was achieved using circular stapler [126]. Early clinical series of transanal TME with laparoscopic assistance (n = 72) were promising, with overall intraoperative and postoperative complication rates of 8.3% and 27.8%, respectively, similar to laparoscopic TME [127].…”

“…Ad hoc instrumentation for visceral closure has been developed with promising results. The over-the-scope clip (OTSC® System; Ovesco Endoscopy AG, Tübingen, Germany) probably remains the most reliable method for closure of the luminal access site in animal models [69,70,71,72,73]. Endoscopic application of T-bars has also been evaluated in the setting of viscerotomy closure [73,74], whereas their efficiency has been questioned by experimental studies [75,76].…”
