Pure Red Cell Aplasia due to B19 Parvovirus Infection after Autologous Stem Cell Transplantation

Abstract: Parvovirus B19 is recognized as a rare cause of pure red cell aplasia (PRCA) in allogeneic stem cell (SCT) and solid organ transplant patients. We report a patient with Hodgkin's disease who developed PRCA due to parvovirus B19 after autologous SCT and who had an excellent response after treatment with gamma-globulin.

“…The time of the onset of PVB19 disease range from 0.03 to 96 months after HSCT. Rash and cytopenia are the main manifestations of PVB19 infection (26/32 cases) [12][13][14][15][16], followed by other symptoms including arthralgia, malaise, headache, myalgia, and pruritus [17][18][19][20][21][22][23][24]. Four of the 32 patients developed organ failure involving the heart, the lungs, or the kidneys [21,24],among which three patients also had cytopenia.…”

“… 23 In a group of 201 patients after BM transplantation, the incidence of B19V infection was low (1.5%), and anemia was the main clinical manifestation observed. 62 Other few cases were also reported, 34 , 36 - 41 some of them linked to transmission by platelet transfusion, erythrocytes concentrate infusion or to acute infection. Therefore, although B19V appears to be transmitted by blood-derived products, hematopoietic allografts do not seem to be a source of frequent virus transmission.…”
