Quality Improvement in the Coagulation Laboratory: Reducing the Number of Insufficient Blood Draw Specimens for Coagulation Testing

Abstract: Our evidence strongly suggests that expired VBCTs consistently and progressively yield QNS specimens. Methods to reduce blood draws from expired VBCTs may include communications promoting proper blood draw technique, floor sweeps to remove expired VBCTs, and improved inventory management.

“…Environmental factors such as temperature, humidity, altitude and light exposure can have a significant impact on the quality of the blood collection equipment. Pre-evacuated blood collection tubes which are beyond the expiry date have a decreased vacuum which may lead to drawing a less than optimal volume of blood and lead to an improper blood to additive ratio [36,37]. Moreover, expired tubes may suffer from some chemical deterioration of the tube additive.…”

“…Prothrombin time may drop down because of lower concentration of fibrinogen; aPTT may lengthen or shorten depending which process prevails: decrease of fibrinogen or activation of coagulation. Haemolysis may distort other coagulation tests results, for instance decrease of antithrombin level [19].…”

“…Coagulation laboratory must regular review how to improve preanalytics in medical institution. Bostic et al [19] studied, how possible to reduce quantity-not-sufficient specimens (QNS) via several methods and to measure effect of expired collection tubes on the amount of blood that can be drawn. During study period the rate of QNS specimens dropped from 0.7 to 0.3%.…”
