Quantification of 4-Beta-Hydroxycholesterol in Human Plasma Using Automated Sample Preparation and LC-ESI-MS/MS Analysis

Abstract: It has recently been proposed that plasma levels of 4β-hydroxycholesterol (4βHC) may be indicative of cytochrome P450 3A4 (P450 3A) activity and therefore could be used to probe for P450 3A-mediated drug-drug interactions. With this in mind, we describe a highly sensitive and precise liquid chromatography-electrospray ionization-tandem mass spectrometry method for the measurement of 4βHC in human plasma with a lower limit of quantification established at 2 ng/mL using 50 μL of plasma. The entire sample prepara… Show more

“…To the best of our knowledge, plasma concentration of 4 ␤ -hydroxycholesterol in patients with renal failure has not been reported previously, which makes comparison impossible. However, our levels are within the range of 18.8-77.0 ng/ml reported by other groups in healthy volunteers ( 18,21,23,25,34,35 ).…”

“…DHEA, dehydroepiandrosterone. 4b-hydroxycholesterol and occasionally the 4b-hydroxycholesterol/ cholesterol ratio have been studied after rifampin induction (Goodenough et al, 2011;Björkhem et al, 2013;Shin et al, 2013) and with inhibitors such as ketoconazole (Goodenough et al, 2011;Shin et al, 2013). Overall, rigorous validation of 4b-hydroxycholesterol as a CYP3A marker has been yet to be performed.…”

“…Among them, 4b-hydroxycholesterol was first reported to be formed solely by CYP3A4 (Bodin et al, 2001). Markedly elevated concentrations of 4b-hydroxycholesterol were found in patients treated with CYP3A4 inducers (Niemi et al, 2006;Josephson et al, 2008;Wide et al, 2008;Diczfalusy et al, 2009;Goodenough et al, 2011), and decreased concentrations have been detected in patients treated with CYP3A4 inhibitors (Josephson et al, 2008;Lütjohann et al, 2009;Goodenough et al, 2011). Furthermore, a relationship between blood 4b-hydroxycholesterol concentration and the number of active CYP3A5*1 alleles has been demonstrated, suggesting that 4b-hydroxycholesterol is not only formed by CYP3A4, but also by CYP3A5 .…”
