Quantifying comorbidity in peritoneal dialysis patients and its relationship to other predictors of survival

Abstract: Comorbidity has a quantitative effect on survival that is independent of age, RRF and membrane function in PD patients. Comorbidity also appears to be associated with increased solute transport at the start of treatment, which is sustained. With the exception of diabetes, grade of comorbidity does not have a profound effect on loss of RRF.

“…4,16,17 Patients on PD have received considerable attention due to the association of PD with a number of comorbidities, including viral hepatitis, coronary heart disease, DM, and HTN. Therefore, the comorbidity indexes as a predictor of mortality are more important than a single comorbidity.…”

“…At the initiation of PD, the comorbidities were graded by the Davies index. 4 The comorbidities by the Davies index were ischemic heart disease, peripheral vascular disease, left ventricular dysfunction, diabetes mellitus (DM), systemic collagen vascular disease, and other significant pathologies. As described previously, the Davies index was categorized as low risk (0), intermediate risk (1)(2), and high risk (≥3).…”

“…Comorbidity was documented as described previously and validated by prospective studies (17,18). Seven comorbid domains are considered, including noncutaneous malignancy, ischemic heart disease (IHD), peripheral vascular disease (PVD; including cerebrovascular and renovascular disease), left ventricular dysfunction (LVD; moderate to severe hypokinesia on two-dimensional echocardiogram), diabetes (the current/previous need for oral anti-diabetics or insulin), systemic collagen vascular disease, and any other condition that is known to reduce life expectancy.…”
