Question on clinical efficiency and principles of diode laser radiation activity at a primary open-angle glaucoma

Abstract

The efficiency of a diode laser with a wavelength 0,8 1 microns in treatment of an open-angle glaucoma is investigated. The diode LTP is an pathogenesious oriented method oftreatment of an open angle glaucoma: it gives in improvement of outflow of an intraocular fluid, stabiliration and improvement ofvisual functions for patients with initial and developed stages of disease at term of observation up to 12 mounths.The laser treatment ofthe primary open angle glaucoma during the last two decades has won a worthy place in system of treatment of glaucoma. Such methods are widely known, as the lasergoniopuncture, laser trabeculoplasty, laser cyclotrabeculospasis (2 -7), permitting is differentiated to approach to a choice of a method of treatment of a glaucoma depending on the mechanism of activity of a laser radiation and clinical features of a primary open angle glaucoma.However justified is the searching for of new trajectories in treatment of a primary open angle glaucoma, for example, at laserresistant (for the set forth above procedures) types of a glaucoma. Perspective in treatment of an open angle glaucoma the application of the diode laser (1 1) is, which energy parameters (maximal power up to 1,5 Watts at minimum diameter of a spot in a focal plane 100 microns), and also mechanism of its interaction with tissues of an eye (photocoagulation), allow to assume prospects of its application in clinic, including for traction action on a drainage zone of an angle of an anterior chamber of an eye at an open angle glaucoma. Its high service performance (portability, high efficiency, absence of necessity in water cooling etc.), low commercial cost (in 4-5 times more cheaply argon) falls into number of virtues of the diode laser. Besides in connection with that the radiation of the diode laser is in infrared range of a spectrum, i.e. outside of limits of visual sensitivity of an eye of the person, by operation with this laser there is a blinding effect, that ensures major comfort of laser interference for the patient.The purpose of the present investigation is the study of clinical efficiency and attempt of a justification of the mechanism of activity on the basis of morphological examinations.
