Radiation therapy for advanced and metastatic melanoma

Abstract: Radiation therapy (RT) is an important modality in cancer treatment. However, the general perception is that melanoma is radio-resistant. High quality clinical trials are helping to establish the place of RT in select scenarios of advanced disease at primary, regional, and distant sites. New RT techniques need to be integrated with effective new systemic therapies within a multidisciplinary environment to ensure optimum patient outcomes. It is important that radiation oncologists embrace this opportunity.

“…The latter is called the abscopal effect and has been described by several authors. [38][39][40][41] The suggested mechanism of action is immune-related, as changes in circulating immune cells and increases in antibodies against melanoma antigens were seen in these cases. In addition, some preclinical and clinical studies suggest that radiotherapy can enhance the therapeutic benefit of immunotherapy.…”

“…It is noteworthy, that CAP treatment was effective in tumor cells showing resistance to alkylating agents suggesting that the patients with an acquired glioma cell resistance to alkylating chemotherapy might benefit from the concomitant treatment with the CAP jets . To date, no resistance toward CAP treatment has been reported, supporting the notion that the direct use of CAP jet exposure as well as the use of the CAP‐activated media could be the promising tools in anti‐cancer therapy, especially as adjuvant treatments in a combination with the specific chemo‐ and radio‐ and even nano‐anti‐cancer therapies . Translation of in vitro results into clinical application is demanding and, therefore, further investigations of CAP effects on tumorigenic and non‐tumorigenic brain cells in vitro, as well as in vivo studies in eligible animal models need to be carried out.…”

“…Common indications for palliative RT include pain, mass effect, tumorrelated hemorrhage, and local irritation from skin or subcutaneous lesions (65). New RT techniques, such as SRS and SBRT, can achieve high probability of local control with very limited toxicity.…”
