Randomised trial comparing propranolol with atenolol in immediate treatment of suspected myocardial infarction.

Abstract: Summary and conclusionsThe value of beta-blockade for suspected acute myocardial infarction was assessed by determining the sixweek and one-year mortality rates in patients started on propranolol, atenolol, or placebo immediately on entry to a coronary care unit. A total of 388 patients entered this double-blind, randomised study, and when analysed on the basis of the initial, intention-to-treat categories there was no significant difference between the three groups in respect of the mortality rate at one year… Show more

“…Características dos estudos selecionados para avaliação da eficácia dos betabloqueadores no IAM. 34 Nos pacientes tratados dentro de sete horas do iní-cio do IAM, no estudo MIAMI, a ocorrência de um novo IAM definido foi menor nos pacientes que receberam succinato de metoprolol (70,1%) do que no grupo controle (74,0%). O grupo COMMIT, por sua vez, relatou redução relativa de 15 a 20% no grupo succinato de metoprolol.…”

“…All patients studied had initially been admitted to coronary care units within 24 h after a myocardial infarction and subsequently attended an outpatient clinic whilst taking part in a study of the effects of long term administration of a ,B-adrenoceptor antagonist after myocardial infarction (Wilcox et al, 1980). The treatment regimens were either, propranolol 80 mg twice daily, atenolol 50 mg twice daily or placebo tablets twice daily.…”

“…Moreover, the infl uence is about the same in both groups and remains statistically signifi cant even after adjustment for baseline characteristics. Thus, attempts to infer treatment efficacy by restricting attention to compliant members of the treatment group or by comparing compliant and noncompliant subjects can be deceptive (136).…”
