“…Thus, the cell-fate transition might represent a mechanism for lung ADC developing resistance towards YAP-targeted therapy. In line with this, multiple reports also show that a phenotypic transition from ADC to small cell lung carcinoma (SCLC) promotes the drug resistance to EGFR tyrosine kinase inhibitors 47,48 . For these complicate diseases, tumour-type-specific and/or combinatory targeting strategies might be required to achieve more effective clinical outcomes for YAP-targeted therapy, especially to LKB1-mutant lung cancer.…”

“…These conflicting findings suggest the possible existence of factors other than EGFR inhibition that might promote the transformation from EGFR-mutant adenocarcinoma to SCLC (46). In this case, in addition to the poor response to TKIs, the increased concentration of serum neuron-specific enolase, which rose from 17.9 ng/mL at the early stage of the disease to 211.10 ng/mL at the stage when progression was detected (reference range <15 ng/mL), could be a way to predict potential disease transformation (71).…”

“…Zhang et al (71) reported the case of an 80-year-old man with lung adenocarcinoma (stage IB) who had an EGFR mutation (deletion of exon 19). Second-line treatment with EGFR-TKI after progression failed, and the progression was accompanied by increased concentrations of the serum tumor marker neuron-specific enolase.…”

“…It is occasionally difficult to perform a second biopsy, and so NSE level may be useful for the early detection of transformation to SCLC in cases that are resistant to EGFR-TKI therapy. NSE could potentially overcome the limitations of performing biopsies on single lesions, which may miss the transformation of another metastatic lesion into SCLC (21)(22)(23)(24). It was concluded that a secondary biopsy was important for the evaluation of genetic and histological changes and the selection of an appropriate treatment following TKI resistance, and that NSE may be useful for the early detection of SCLC transformation.…”
