Real-Time Sonoelastography: Findings in Patients with Symptomatic Achilles Tendons and Comparison to Healthy Volunteers

Abstract: Our results emphasize that only distinct softening of Achilles tendons is comparable to clinical examination and US findings. However, mild softening might be explained by very early changes in tissue elasticity in the case of Achilles tendinopathy, which should be assessed in follow-up studies.

“…These histopathologic alterations of tendinopathy have been found to increase the compressibility of tissue on ultrasound scans and may cause softening of the tissue on SE [33]. De Zordo et al reported that SE is valuable in the detection of intratendinous and peritendinous alterations in lateral epicondylitis or Achilles tendinopathy [23,34]. In this study, distinct focal lesions were detected with SE and scored as grade 0 in 7.6 % of cases, grade 1 in 30.5 %, grade 2 in 19.5 %, and grade 3 in 42.4 %.…”

“…As musculoskeletal disorders are associated with abnormalities in the muscle and tendon biomechanics, sonoelastography may have potential clinical applications in the musculoskeletal system. The Achilles tendon has provided most of the clinical data available so far for musculoskeletal applications [5][6][7].…”

“…Sonoelastographic grading was performed on the basis of cine loops of the compression-relaxation cycle to ensure reproducible results. We concentrated on the middle of each cycle, since the most reliable results are provided during this phase (15). In addition, the position of B-mode US and sonoelastography are not considered 100% comparable.…”
