Reconsidering the minimally important difference: evidence of instability over time and across groups

Abstract: BACKGROUND CONTEXT: Underlying cognitive factors have been found to influence patients' symptom experience. Current evidence suggests that concomitant changes in appraisal must be taken into account to accurately interpret change as measured by standard spine patientreported outcomes (PROs). PURPOSE: To investigate changes in patients' minimally important differences (MID) over recovery from spinal surgery; whether and how cognitive appraisal processes are implicated in the change trajectories. “…Rather, the best predictors of positive‐emotions change were measures of appraisal processes . These ways of thinking about quality of life (QOL) have been implicated in response‐shift studies in people with HIV/AIDS, multiple sclerosis, spine surgery, bladder cancer and heterogeneous rare conditions . In our longitudinal study of haemophilia caregivers, positive‐emotions change was associated with prioritizing staying positive and a reduced emphasis on interpersonal problem‐solving …”

“…25 Variability in the MID was also noted by Schwartz et al in a study of 167 patients undergoing spinal surgery. 62 The authors investigated changes in patients' MID over recovery and how cognitive appraisal processes were implicated in the change trajectories. 62 Standard spine outcome measures were used, along with the QOLAP Standards of Comparison items.…”
