Reconstruction for DRUJ Instability

Abstract: The skeletal architecture of the DRUJ provides minimal inherent stability, as the sigmoid notch is shallow and its radius of curvature is 50% greater than that of the ulnar head [Af Ekenstam F, Hagert CG. Anatomical studies on the geometry and stability of the distal radio ulnar joint. Scand J Plast Reconstr Surg 1985;19(1):17-25]. Due to its incongruent articulation, the DRUJ relies strongly on the surrounding soft tissues for stability. The triangular fibrocartilage complex (TFCC) is generally accepted as th… Show more

“…groups have their upper arm immobilized in mild supination, depending on the most stable position that was determined during the manual stress test. 20 During the immobilization period, all patients began finger motion exercises to control edema. Active wrist supination and pronation exercises were initiated after splint removal, and the patients were followed-up at 1, 3, 6, and 12 months.…”

“…Most investigators agree that the TFCC is the primary stabilizer of the distal radioulnar joint. 22,30,31 With the TFCC intact, 2 forces are registered at the load cell: DRUJ contact force and tension in the TFCC. When the TFCC is divided, this 1 component of force is removed, causing a decrease in the load at the distal ulna.…”
