Redefining Relative Biological Effectiveness in the Context of the EQDX Formalism: Implications for Alpha-Particle Emitter Therapy

Abstract: Alpha-particle radiopharmaceutical therapy (αRPT) is currently enjoying increasing attention as a viable alternative to chemotherapy for targeting of disseminated micrometastatic disease. In theory, αRPT can be personalized through pre-therapeutic imaging and dosimetry. However, in practice, given the particularities of α-particle emissions, a dosimetric methodology that accurately predicts the thresholds for organ toxicity has not been reported. This is in part due to the fact that the biological effects caus… Show more

“…This problem can be potentially addressed using physiologically-based voxel tissue models (i.e., cell-level) such as MIRDcell which are needed for cellular and multicellular level dosimetry (34). Accordingly, understanding the implication of normal organ or tumor dosimetry from an α emitter requires knowledge of the relative biological effectiveness (RBE) (35).…”

“…The RBE is a concept that determines the ratio of absorbed doses, to attain a given biological response [35]. RBE values were calculated by comparing the physical dose (D 60 Co/D 99m Tc-C 3 /C 5 ) that results in 50% survival (RBE 0.5 ) and survival (SF 60 Co/SF 99m Tc-C 3 /C 5 ) for a 2 Gy dose (RBE2Gy).…”

“…It must be referred that for 60 Co and 99m Tc-C 3 /C 5 the modeling of the SF vs absorbed doses was made using the linear model described in [35] and detailed in SI.…”
