Reduced Morbidity and Improved Healing with Bone Morphogenic Protein-2 in Older Patients with Alveolar Cleft Defects

Abstract: For this select group of late-presenting alveolar cleft patients, the BMP-2 procedure resulted in improved bone healing and reduced morbidity compared with traditional iliac bone grafting.

“…The cost offsets were attributable to a reduced hospital stay, reduced operative costs, as well as reduced costs of outpatient attendances. A reduced hospital stay associated with the use of BMPs compared to ICBG has been reported by other authors [8]. The operative costs have been related to better use of theatre time, staff, and equipment in group 2.…”

“…In particular, BMP-2, which possesses potent osteoinductive activity, has been extensively studied 2) . Some reports have demonstrated that recombinant human (rh) BMP-2 with absorbable collagen sponges (ACS) can induce new bone formation in mandibular defects and cleft palate defects, with results comparable to autogenous bone grafts 2,3) . However, the supraphysiological doses of rhBMP-2 required for osteoinductive effects in humans may produce adverse effects 4,5) , including edema, erythema and gingival swelling.…”

“…The ideal scaffold materials should be resorbed over time to allow cell infiltration and extracellular matrix deposition for new bone formation, while maintaining its osteoconductive properties until they are no longer required [17][18][19] . So far, ACS has been extensively used as an appropriate carrier for BMPs in bone reconstructive surgery 2,3,20,21) . Conversely, Bergeron et al recently reported that the injection of a chitosan-based BMP-9 gel induced ectopic bone formation, whereas collagen-based BMP-9 gel injection failed to promote ectopic ossification in mouse quadriceps 22) .…”

“…Luego en el año 2008 se realizó un ensayo clínico en 21 pacientes esqueletalmente maduros (Dickinson et al, 2008). Los grupos fueron elegidos al azar entre el uso de rhBMP-2 e injerto autólogo de cresta ilíaca; el seguimiento fue por 1 año.…”
