Introduction: Reduction mammaplasty can be a life-changing event for women with symptomatic macromastia. Many women experience improvement in physical symptoms, including resolution of neck, back and shoulder pain. There are other reported positive changes, including more physical activity, improved glucose control and a self-confidence. Adolescents have demonstrated weight loss following reduction mammaplasty but there is little objective data in the adult population. Methods:After obtaining institutional review board approval, eligible patients were identified in our institution's electronic record database, eRecord (Epic; Madison, WI). Eligible subjects were identified using i2b2 software for the diagnosis of macromastia (ICD-9 611.1) and the CPT code for reduction mammaplasty (19318). A retrospective study chart review was performed from June 1, 2011 to time September 30, 2015 for97 women. The mean BMI was calculated at different time intervals post-operatively, including 1 week, 1 month, 3 months, 6 months, 12 months, 1 year, 2 years and 2+ years. The BMI values for each patient were then compared to the 1 week postoperative BMI. The change was then analyzed to determine if there was any significant weight loss. Results:There is a slight increase in BMI with increasing post-operative time (mean of 31.934 at one week compared to 33.255 2+ years) although this is not statistically significant. The lowest 50% of patients based on baseline-BMI was compared with the highest 50% of patients; each group showing no significant difference over time. Similar analyses using a mixed-effects model were performed given the longitudinal nature of the data, however this also showed that there was no significant change. Conclusion:Despite well-document physical and psychological improvements following reduction mammaplasty, there is no statistically significant BMI decrease for women of all BMIs at time points from 1 month to more than 2 years when compared to their 1-week post-op BMI.

Summary: A multidisciplinary work group involving stakeholders from various backgrounds and societies convened to revise the guideline for reduction mammaplasty. The goal was to develop evidence-based patient care recommendations using the new American Society of Plastic Surgeons guideline methodology. The work group prioritized reviewing the evidence around the need for surgery as first-line treatment, regardless of resection weight or volume. Other factors evaluated included the need for drains, the need for postoperative oral antibiotics, risk factors that increase complications, a comparison in outcomes between the two most popular techniques (inferior and superomedial), the impact of local anesthetic on narcotic use and other nonnarcotic pain management strategies, the use of epinephrine, and the need for specimen pathology. A systematic literature review was performed, and an established appraisal process was used to rate the quality of relevant scientific research (Grading of Recommendations Assessment, Development and Evaluation methodology). Evidence-based recommendations were made and strength was determined based on the level of evidence and the assessment of benefits and harms.

Backgrounds The positive effects of reduction mammoplasty on metabolic profile have been shown in a limited number of studies. This study objective to reveal the effects of reduction mammoplasty on metabolic profile and anthropometric measurements. Subjects and MethodThe study was prospectively conducted on 42 patients who were operated between April 2019 and March 2020. Fasting plasma glucose, fasting plasma insulin, total cholesterol, triglyceride, high-density lipoprotein and low-density lipoprotein cholesterol, HgA1c, homeostasis model assessment scores, adiponectin, leptin, and resistin levels were evaluated. In addition, age, height, weight, body mass index; breast, chest, waist, hip circumference; waist-hip ratio, and bilateral breast resection tissue weights were recorded. Data and blood samples were collected one hour before the operation, 6 and 12 weeks after the operation. Result The patients' mean age was 43.14±10.24, and their average height was 159.42±4.96 cm. The excised bilateral dermo fatty tissue weight was 1435.85±721.16 g. At the postoperative 40th day a decrease in leptin (p = 0.001), resistin (p =0.008), glucose (p = 0.021) and insulin resistance values (p=0.013) stated. There was an increase in adiponectin (p \ 0.001) and HDL (p = 0.013) levels at the postoperative 40th day. In the postoperative third month, these data returned to the previous levels that were measured before operations. However, an increase in hip circumference (p = 0.034) and a decrease in waist-hip ratio (p\0.001) was detected in third month. Also, there was no difference in body mass index and weight compared to preoperation. Conclusion After reduction mammoplasty, compensatory fat growth in the hip area, an increase in the hip circumference, and a decrease in the waist-hip ratio were observed in the postoperative third month.
