“…28 A potential explanation for the results of the present study is that long-term antihypertensive treatment may progressively reduce myocyte hypertrophy (as seen by an improvement in LVM), but may be less effective at preventing fibrosis or changes in cardiomyocyte stiffness. The mild deterioration in diastolic function of adequately treated hypertensive patients between phase 1 and 2 is of a similar magnitude to the expected decline associated with 2 years of ageing, 29,30 but the absence of an untreated hypertensive comparator group (for obvious ethical reasons) prevents any firm conclusion regarding whether the control of BP per se affects the time-dependent decline of diastolic function in hypertension. e′ in the amlodipine±perindopril-based regimen declined to a greater degree between phases which may be because, despite better early evolution of diastolic function in this group because of initial reductions in fibrosis, ageing and the associated changes still ultimately intervene.…”
