Refractory reverse amblyopia with atropine penalization

Abstract: Pharmacological penalization with atropine has been shown to be equally effective as conventional occlusion therapy in the treatment of amblyopia in children. Reverse amblyopia of the sound eye with atropine penalization has been reported before, but is more common in cases where the effect is augmented with optical penalization and is mostly reversible. We report a case of reverse amblyopia with atropine penalization, in a 4-year-old girl, which was refractory to treatment. This report highlights the need for… Show more

“…However, there are rare cases in which the initially dominant eye may be left with lesser vision. [ 8 9 10 ] Levodopa, when not combined with patching/penalization, doesn’t cause occlusion amblyopia because both eyes are stimulated. Similarly, part time patching is also not reported to cause occlusion amblyopia whether used with or without adjunctive levodopa.…”

“…Pharmacological penalization is obtained by the use of cycloplegic drugs such as atropine. 57,58 Optical penalization requires wrong lenses deliberately worn on the good eye to encourage the use of the amblyopic one and has a good therapeutic efficacy in mild to moderate amblyopia.…”

“…This involves patching the dominant/healthy eye for 2–6 h/day, depending on the severity of the amblyopia, every day for several months up to years [ 9 ]. However, occlusion therapy carries the risk of reverse amblyopia and new strabismus by overtreatment [ 10 ]. In addition, poor adherence to the occlusion regimen is a common problem in pediatric populations (ranging from 49 to 87%), resulting in treatment failure [ 11 ] and residual amblyopia (i.e., reduced visual acuity and stereopsis that will persist into adulthood).…”
