Relapse in schizophrenia: a review of the concept and its definitions

Abstract: The increasingly rigorous attention paid to the diagnosis of schizophrenia has not yet been extended to the development of reliable and valid criteria for describing the course of the illness. Over the past two decades the concept of 'relapse' has become the pre-eminent measure of the success of community treatment programmes of schizophrenia. However, to date there has been little effort to examine this measure critically, or to develop other measures to reflect the wide range of clinical outcome in schizophr… Show more

“…This study provides a novel approach to operationally defining relapse by using a combination of telephone interviews and casenotes. While numerous relapse definitions exist, these rely on face-to-face assessments or solely on casenotes (Eisner et al, 2013; Falloon et al, 1983; Gleeson et al, 2010; Olivares et al, 2013). We suggest that remote relapse assessment is more easily integrated into participants' lives, decreasing both participant and researcher burden; this is likely to increase engagement and allow more frequent, long-term monitoring.…”

“…Si la rechute est une composante importante du cours de la schizophrénie, sa nature dichotomique ne saurait être un reflet fidèle de l'éventail complet des degrés fonctionnels dans la schizophrénie. De même, l'utilisation de la rechute comme mesure de l'évolution de la maladie exagère l'importance d'un bref épisode difficile dans la vie du patient (Falloon, 1983). L'absence de rechute n'est synonyme ni de santé ni de qualité de vie satisfaisante pour le patient.…”

“…While the family may serve a useful purpose in identifying and supporting the in dividual in times of crisis, perhaps other alter natives to hospital admission could be presented to the family at such times. Conversely, hosti lities within the family environment could be instrumental in the individual's continuing hospital admission, and intervention should address this problem, particularly if admissions represent crisis or intolerance within the family, rather than a patient's illness (Falloon et al 1983).…”
