Relationship between a comprehensive social determinants of health screening and type 2 diabetes mellitus

Abstract: Highlights Most social needs were more commonly reported in patients with T2DM. Prescription cost and transportation had the strongest association with T2DM. Patients diagnosed with T2DM were 1.7X more likely to report any social need.
