Background: Knee osteoarthritis is a chronic joint illness distinguished by the gradual cartilage loss of extracellular matrix and subsequent disturbance in chondrocyte biology. Decorin is a small leucin-rich proteoglycan that provides physical linkage among both molecules of aggrecan and collagen II fibrils. It is an important structural component of matrix integrity and cartilage biomechanical function. Its level helps to determine OA grading and subsequently management.Objective: to measure the serum decorin levels in knee OA patients and to explore their association with clinical manifestations and disease severity. Methodology:In this case-control study, 50 patients with varying grades of knee OA were compared to 50 age-and sexmatched seemingly healthy people as controls. The serum decorin level in both groups has been measured using the enzyme linked immunosorbent assay (ELISA) technique.Results: There had been a highly statistically significant elevation in serum decorin in the OA patient group, with a median of 52.4 (9.8-66.8)compared to 8.6 (6.6-9.13)in the control group, and there was a significant positive correlation (p<0.05) between decorin level and Western Ontario and McMaster Universities (WOMAC). Conclusion:Serum decorin has been significantly elevated in the patient group and its level correlates positively with WOMAC.

Proteoglycans are vital components of the extracellular matrix in articular cartilage, providing biomechanical properties crucial for its proper functioning. They are key players in chondral diseases, specifically in the degradation of the extracellular matrix. Evaluating proteoglycan molecules can serve as a biomarker for joint degradation in osteoarthritis patients, as well as assessing the quality of repaired tissue following different treatment strategies for chondral injuries. Despite ongoing research, understanding osteoarthritis and cartilage repair remains unclear, making the identification of key molecules essential for early diagnosis and effective treatment. This review offers an overview of proteoglycans as primary molecules in articular cartilage. It describes the various types of proteoglycans present in both healthy and damaged cartilage, highlighting their roles. Additionally, the review emphasizes the importance of assessing proteoglycans to evaluate the quality of repaired articular tissue. It concludes by providing a visual and narrative description of aggrecan distribution and presence in healthy cartilage. Proteoglycans, such as aggrecan, biglycan, decorin, perlecan, and versican, significantly contribute to maintaining the health of articular cartilage and the cartilage repair process. Therefore, studying these proteoglycans is vital for early diagnosis, evaluating the quality of repaired cartilage, and assessing treatment effectiveness

Decorin is an extracellular matrix protein that belongs to the family of small leucine-rich proteoglycans. As a matrix protein, the first discovered role of decorin is participating in collagen fibril formation. Many other functions of decorin in various biological processes have been subsequently identified. Decorin is involved in an extensive signaling network and can interact with other extracellular matrix components, growth factors, receptor tyrosine kinases, and various proteases. Decorin has been shown to be involved in wound repair, cell cycle, angiogenesis, tumor metastasis, and autophagy. Recent evidence indicates that it also plays a role in immune regulation and inflammatory diseases. This review summarizes the characteristics of decorin in immune and inflammatory diseases, including inflammatory bowel disease (IBD), Sjögren’s syndrome (SS), chronic obstructive pulmonary disease (COPD), IgA nephropathy, rheumatoid arthritis (RA), spondyloarthritis (SpA), osteoarthritis, multiple sclerosis (MS), idiopathic inflammatory myopathies (IIM), and systemic sclerosis (SSc) and discusses the potential role in these disorders.
