Breast cancer is a complex disease with variable molecular characteristics and shows different tumor behavior, therapeutic response, and prognosis. The regional general hospital Madiun has begun to improve services for breast cancer patients since 2015 with the availability of immunohistochemical examinations. This effort was made because breast cancer therapy is currently guided by these examinations. This study aimed to determine the distribution of various molecular subtypes of breast cancer in Madiun Hospital and also the relationship between molecular subtypes and histopathological grading. This study was a retrospective study with a cross-sectional design taken from the Anatomical Pathology Laboratory of Madiun Hospital data from 2015 to 2018. A total of 281 breast cancer cases were examined for immunohistochemical of estrogen receptor (ER), progesterone receptor (PR), and human epidermal receptor 2 (HER2), and a total of 91 breast cancer patients plus Ki-67 examination were then classified into luminal A, luminal B, HER2 and triple-negative (TN). The statistical test used was the Chi-Square test with a 95% confidence level. The most common molecular subtypes of breast cancer in Madiun Hospital were luminal B (28,5%) followed by TN (27,5%), luminal A (22%), and HER2 (22%). The mean age of breast cancer patients was 52.8 ± 10.57. The most common histological types and grades of breast cancer were invasive ductal carcinoma (85.1%) and tumor grade 1 (58.71%). There was a correlation between the immunohistochemical features of ER, PR, HER2, and Ki67 withhistopathological grading. Positive ER and PR were more well differentiated but e HER2 positive andKi67 were more poorly differented. Abstrak Kanker payudara merupakan penyakit kompleks dengan gambaran molekuler bervariasi dan menunjukkan perilaku tumor, respon terapi, dan prognosis yang berbeda. Rumah Sakit Umum Daerah (RSUD) Madiun mulai meningkatkan layanan untuk pasien kanker payudara sejak tahun 2015 dengan tersedianya pemeriksaan imunohistokimia. Upaya ini dilakukan karena terapi kanker payudara saat ini berpedoman atas pemeriksaan tersebut. Penelitian ini bertujuan untuk mengetahui distribusi berbagai subtipe molekuler kanker payudara di RSUD Madiun dan juga hubungan antar subtipe molekuler dengan grading histopatologi. Penelitian ini adalah studi retrospektif dengan desain cross sectional yang diambil dari data Laboratorium Patologi Anatomi RS Madiun selama tahun 2015 sampai 2018. Total 281 kasus kanker payudara dilakukan pemeriksaan imunohistokimia estrogen receptor (ER), progesteron receptor (PR) dan human epidermal epidermal receptor 2 (HER2) dan total 91 pasien kanker payudara ditambah pemeriksaan Ki-67 kemudian diklasifikasikan menjadi luminal A, luminal B, HER2, dan triple negative (TN). Uji statistik yang digunakan adalah uji Chi Square dengan taraf kepercayaan 95%. Subtipe molekuler kanker payudara di RS Madiun yang paling banyak adalah luminal B (28,5%) diikuti TN (27,5%), luminal A (22%), dan HER2 (22%). Rata-rata umur pasien kanker payudara adalah 52,8 ±10,57. Tipe histologi dan grade kanker payudara terbanyak adalah karsinoma duktal invasif (85,1%) dan grade tumor 1 (58,71%). Terdapat hubungan gambaran imunohistokimia baik ER, PR, HER2 dan Ki67 dengan grading histopatologi. ER dan PR positif lebih banyak mengalami diferensiasi baik namun HER2 positif dan KI67 positif lebih banyak mengalami diferensiasi buruk.

BACKGROUND: Animal models with mammary cancer that closely mimic human breast cancer for treatment development purposes are still required. Induction of 7,12-dimethylbenzanthracene (DMBA) to rats shows the histopathological features and mammary cancer characterization similar to humans. Examinations of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), and Ki67 expressions are crucial in deciding the treatment and prognosis of breast cancer. AIM: This research aimed to view histopathology images of mammary glands and expressions of ER, PR, Ki67, and HER2 of DMBA-induced rats. METHODS: After 1-week adaptation, 11 5-weeks-old female rats were induced with 20 mg/kg body weight (BW) of DMBA 2 times a week for 5 weeks. On week 29, nodules taken from the mammary gland were examined for hematoxylin-eosin staining and immunohistochemistry with p63, ER, PR, HER2, and Ki67 antibodies. The grading score used the Nottingham Grading System and molecular classifications based on St. Gallen 2013. RESULTS: Six rats had nodules, but the histopathologic features of one nodule showed normal mammary gland without cancer. The histopathological type of mammary cancer was cribriform carcinoma, comedo carcinoma, lipid-rich carcinoma, adenocarcinoma squamous, and adenomyepithelioma. Histopathological grading showed 60% of grade 3 and 40% of grade 2. P63 expression showed 60% positive and 40% negative. The frequency of ER, PR, HER2, and Ki67 of five nodules showed positivity: 40%, 60%, 60%, and 60%, respectively. Molecular subtypes of Luminal A, B, HER2, and triple-negative were 0%, 60%, 20%, and 20%, respectively. CONCLUSION: Histopathological features and molecular subtype of mammary cancer on rats induced with 20 mg/kg BW of DMBA showed similarity to human breast cancer.

Background Breast cancer (BC) remains a significant global concern, particularly among developing countries in South-East Asia (SEA) and South America (SA). The socioeconomic burdens of oncologic care in those countries were often originated from limited accessibility on attainable therapeutic options and reliability on identifying essential information of cancer cells, i.e., immunohistochemical (IHC) subtyping to determine suitable approaches. The triple-negative breast cancer (TNBC) is among the most aggressive category in breast malignancy, therefore, requiring more specific molecular pathway blocking to exhaust the cells. However, large-scale epidemiological investigation on its rate among BC remains unavailable to date. This study aimed to describe the prevalence of TNBC in the SEA and SA continents since it may guide the future direction of oncologic research and trials. Methods This review focuses on observational studies from the SEA and SA continents from the last decade. Each study represents its country or cities, period of observation, population size, and the TNBC-BC rate as the main outcomes. Therefore, we may also limit the reporting bias originated from same-patient data on the specific occasions. The analysis will be derived to SEA-SA comparison, plus SEA/SA-specific session as processed in Comprehensive Meta-Analysis (CMA) version 3.0. The statistical analysis will be performed in random effects model (REM) within 95% confidence interval (CI). Results From 46 studies included in the final analysis with a total enlisted population of 34,346 unique individuals with BC, the TNBC rate was higher in the SEA compared to the SA region (19.3% vs. 15.7%; P < 0.05 in 95% CI), with the highest prevalence observed in Vietnam (22.4%) and Peru (17.8%), if it was restricted on countries with two or more studies. Interestingly, both Laos and Argentina possessed significant differences compared to other countries within their respective continents, with the highest and lowest TNBC rates (P < 0.05). Conclusions The IHC characteristics in SEA differ from those in the SA continent as mainly represented by TNBC prevalence, possibly shaping the course of future trials in the respective region based on IHC expressivity status.
