Relationship between Urological Disease and Inferior Vena Cava Reflux on Color Doppler Ultrasonography

Abstract: IVC reflux may be positively or negatively related to the occurrence of some urological diseases. Pelvic congestion secondary to IVC reflux may be one of the factors contributing to chronic prostatitis and stress incontinence.

“…The prevalence of tricuspid regurgitation is very high, and reaches 70% among adults in Japan, whereas it is 17–68% in patients with otherwise “normal” hearts from the USA . Our previous ultrasonographic study showed that the prevalence of inferior vena cava reflux associated with tricuspid regurgitation in the supine position was significantly higher in women (68%) than in men (49%) attending our urological outpatient clinic . Chronic prostatitis in men and stress incontinence in women are related to inferior vena cava reflux, and there is also a possible relationship of urethral syndrome and cystocele with inferior vena cava reflux in women .…”

“…The causes of pelvic venous congestion include compression of the left renal vein (nutcracker phenomenon) into which the left gonadal vein flows, stenosis or obstruction of the internal pudendal vein and tricuspid regurgitation . The prevalence of tricuspid regurgitation is very high, and reaches 70% among adults in Japan, whereas it is 17–68% in patients with otherwise “normal” hearts from the USA .…”

“…Furthermore, when plasma was added dropwise to the bladder in a rat model, the maximum bladder contraction pressure increased, while the interval between the bladder contractions shortened (13). In a tranilast-induced interstitial cystitis rat model, carbazochrome increased both the locomotor activity and the interval between the bladder contractions (16), with benign prostatic hyperplasia (10) or intact female rats (28), naftopidil decreased plasma catecholamine levels and increased serotonin levels. In the present study, naftopidil also showed same results even in rats with pelvic venous congestion.…”

“…Our previous study demonstrated that chronic prostatitis and stress urinary incontinence were related to the existence of vena cava reflux caused by tricuspid regurgitation (28). When we examined a female case with pelvic congestion syndrome using external iliac venography, we found that pelvic venous congestion occurred with tricuspid regurgitation and without pelvic venous valves (25).…”
