REM sleep without atonia in middle‐aged and older psychiatric patients and Lewy body disease: a case series

Abstract: Rapid eye movement sleep without atonia itself may be associated with specific clinical profiles, even when dream-enactment behavior is absent. Continued follow-up of the patients with MDD exhibiting RWA is warranted to determine if they represent the prodromal Parkinson's disease/dementia with Lewy bodies. “…Occipital hypoperfusion, one of the supportive features of the clinical diagnostic criteria for DLB, was observed in a subset of patients in both groups. These were consistent with the finding that occipital hypometabolism/hypoperfusion is commonly associated with clinical features of LBD regardless of cognitive conditions . Although the age of RBD onset was not identified in one patient, the estimated mean age of that in the other patients with RBD symptoms was 64.7 (±11.5) years.…”

“…These were consistent with the finding that occipital hypometabolism/ hypoperfusion is commonly associated with clinical features of LBD regardless of cognitive conditions. 10,31,32 Although the age of RBD onset was not identified in one patient, the estimated mean age of that in the other patients with RBD symptoms was 64.7 (AE11.5) years. Two patients with a clinical history of MDD developed RBD symptoms during a long clinical course, while RBD symptoms preceded the first visit to a psychiatric outpatient service by years in the other six patients.…”

“…Idiopathic RBD (iRBD) is now known as a prodromal symptom of various neurodegenerative disorders, including PD and depression. Moreover, concomitant iRBD and depression increase the likelihood of progression to PD, thereby underscoring pathomechanics overlap/interaction of depression and REM sleep dysfunction in neurodegeneration (Fujishiro et al, 2017). We evaluated structural brain changes in white matter tracts of depressed versus nondepressed PD patients, without underlying RBD or RBD as comorbidity, to manually control for effects of concomitant RBD and depression on structural brain connectivity in early PD.…”

“…In previous studies on patients with PD, constipation was already associated with the presence of RBD and depression . These non‐motor symptoms have been demonstrated to occur many years before the onset of motor symptoms of PD, and are referred to as a condition called ‘pre‐motor PD’ .…”
