Remission in ankylosing spondylitis treated with anti-TNF-  drugs: a national multicentre study

Abstract: Our real-life study on PR confirmed the effectiveness of ADA, ETA or INF as first or second anti-TNF-α drugs. The presence at baseline of enthesitis or psoriasis or low CRP values yielded a lower probability of obtaining PR.

“…In patients with earlyonset axSpA without evidence of radiographic sacroiliitis, MRI can, in fact, detect inflammatory lesions before bone damage becomes visible. As it has been seen that these patients respond quickly and effectively to anti-tumor necrosis factor-α drugs (3,(9)(10)(11), it has become urgent to identify early stages of the disease in order to begin appropriate treatment as early as possible. The Assessment of SpondyloArthritis International Society (ASAS) has recently established classification criteria for patients with suspected axSpA, including those with and without radiographic sacroiliitis (5).…”

“…However, the baseline physical function remained a significant factor associated with HAQ over time, regardless of treatment. SpA patients on anti-TNF are expected to have good functional outcomes[44], and a short delay in diagnosis, male gender and preserved physical function were predictors of minimal disease activity (MDA) at the 5-year follow-up in a early PsA cohort[45]. However, we were not able to demonstrate the effect of anti-TNF in modulating the physical function at 6 year.…”

“…n CONCLUSIONS Anti-TNF therapy showed its efficacy and safety in different rheumatic diseases and now a state of remission or low disease activity are achievable targets even in spondyloarthritis in general (42,43) and in PsA (44)(45)(46). However, about 40% of patients lack to respond to TNF inhibitors.…”
