Background and Objectives Implementation of automated steps in preparing blood components for transfusion from whole blood collections has produced improvements in multiple fields. The aim of this review is to summarize data from existing literature related to automation of whole blood processing systems. Materials and Methods We searched MEDLINE for studies comparing semi‐automated and fully automated whole blood processing systems published before 20 July 2021. Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) guidelines were followed. Additionally, we performed a manual search. Results We identified 500 studies, of which 459 (92%) did not meet the eligibility criteria, and finally 17 studies were included in the analysis. Manual search included six additional studies. Publication year ranged from 2004 to 2021. Automation reduced the run‐time (from 92 to 76 min), improved recovery of haemoglobin in red cell concentrates (RCCs) and resulted in higher red blood cell and platelet yields. Automation also reduced discard rates due to whole blood bag ruptures (1.2%–0.1%), low volume of RCCs (<200 ml; 0.5%–0.03%) and haemolytic plasma (2.1%–0.6%). Automation could reduce the number of full‐time equivalent (FTE) operators or maintain the number of FTE operators while performing additional procedures, and it reduced to 1.13 m2 the space required for the device. Conclusion Automation of whole blood processing resulted in continued improvements in productivity, product quality and technical features. However, too few publications are available to reach strong conclusions. Therefore, it is necessary to expand the scientific knowledge in this field.

Background Monkeypox virus has recently emerged from endemic foci in Africa and, since October 20, 2022, more than 73,000 human infections have been reported by the CDC from over 100 countries that historically have not reported monkeypox cases. The detection of virus in skin lesions, blood, semen, and saliva of infected patients with monkeypox infections raises the potential for disease transmission via routes that have not been previously documented, including by blood and plasma transfusions. Methods for protecting the blood supply against the threats of newly emerging disease agents exist and include Pathogen Reduction Technologies (PRT) which utilize photochemical treatment processes to inactivate pathogens in blood while preserving the integrity of plasma and cellular components. Such methods have been employed broadly for over 15 years, but effectiveness of these methods under routine use conditions against monkeypox virus has not been reported. Study design and methods Monkeypox virus (strain USA_2003) was used to inoculate plasma and whole blood units that were then treated with riboflavin and UV light (Mirasol Pathogen Reduction Technology System, Terumo BCT, Lakewood, CO). The infectious titers of monkeypox virus in the samples before and after riboflavin + UV treatment were determined by plaque assay on Vero cells. Results The levels of spiked virus present in whole blood and plasma samples exceeded 103 infectious particles per dose, corresponding to greater than 105 DNA copies per mL. Treatment of whole blood and plasma units under standard operating procedures for the Mirasol PRT System resulted in complete inactivation of infectivity to the limits of detection. This is equivalent to a reduction of ≥ 2.86 +/- 0.73 log10 pfu/mL of infectivity in whole blood and ≥ 3.47 +/-0.19 log10 pfu/mL of infectivity in plasma under standard operating conditions for those products. Conclusion Based on this data and corresponding studies on infectivity in patients with monkeypox infections, use of Mirasol PRT would be expected to significantly reduce the risk of transfusion transmission of monkeypox.

ZusammenfassungVollblutspenden werden nach der Entnahme in Blutspendediensten zu den Blutkomponenten Erythrozytenkonzentrat, Therapeutisches Plasma und Thrombozytenkonzentrat weiterverarbeitet. Die Verarbeitung kann auf verschiedene Weise erfolgen. In diesem Übersichtsartikel erläutern wir die Unterschiede zwischen den beiden weltweit gebräuchlichsten Verfahren, dem Buffy-Coat-Verfahren und dem Plättchen-Reiches-Plasma-Verfahren, und beschreiben das in Deutschland standardgemäß eingesetzte semi-automatisierte Buffy-Coat Verfahren detaillierter. Darüber hinaus werden die verschiedenen Automatisierungsgrade in der Vollblutverarbeitung und die verschiedenen Ansätze zur weiteren Automatisierung beleuchtet. Im Rahmen dessen zeigen wir auf, welche Automatisierungsmaßnahmen in einigen Einrichtungen bereits umgesetzt werden konnten und welche neuen Techniken die weitere Automatisierung in Zukunft entscheidend prägen könnten.
