Reresection of Colorectal Liver Metastasis with Vena Cava Resection

Abstract: The best known treatment of the colorectal liver metastasis is the complete surgical excision with clean surgical margins. However, liver resections sometimes cannot appear technically feasible due to the high number of metastases in the liver, in cases of recurrent resections or invasion of the tumors to the major vascular structures or neighboring organs. Here, we presented a colorectal recurrent liver metastasis invading the retrohepatic vena cava, right adrenal gland, and right diaphragm. En masse resectio… Show more

“…Individual survival outcomes could be extracted in 126 patients [16,17,[19][20][21][22][23][24]26,27,29,30,[32][33][34][35][36][37]40,41,44]. A Kaplan-Meier analysis showed a median OS of 34 months (confidence interval-CI-: 30-40) (Figure 2), and the 1-year, 3-year, and 5-year OS was 71.4%, 19.8%, and 7.1%, respectively (Table 2).…”

“…In recent decades, an increase in the life expectancy of patients with colorectal cancer has been achieved, with screening, early diagnosis, novel chemotherapy agents, and improvements in radiotherapy and in surgical techniques. Despite these improvements, poor prognostic factors continue to include the presence of metastasis, disease recurrence, and advanced localized tumors, 9 reporting that one-half of these patients with these risk factors die in the presence of these neoplasms. 10 Colorectal cancer is one of the most common diseases, with approximately one million new cases and 500,000 deaths annually worldwide, the main cause of death being metastasis.…”

“…The metastases of colorectal tumors occur most frequently in the liver, given the location of this organ in relation to the portal circulation (33). The presence of liver metastases is a unanimous negative prognostic factor for colorectal cancer patients (34,35), even more for those operated in emergency.…”
