Retinoic acid embryopathy

Abstract: Isotretinoin is a retinoid which is derived from Vitamin A. It is indicated for severe cystic acne treatment, but it has been classified as teratogenic. A wide spectrum of birth defects including craniofacial, heart, and nervous system malformations have been described with prenatal exposure to this drug. We report the case of a newborn with a history of prenatal exposure to isotretinoin with craniofacial defects, including left-sided anotia, right-sided microtia, complex congenital heart disease, and central … Show more

“…Retinoic acid (RA) or tretinoin is an oxidative metabolite of vitamin A that exists in two isoforms: cis and trans . A group of birth defects called retinoic acid embryopathy (RAE) result from the oral administration of 13‐ cis ‐RA . Studies have described an RAE consisting of craniofacial, cardiac, thymic, and central nervous system abnormalities .…”

“…Isotretinoin, 13-cis-retinoic acid, has been extensively prescribed for the systemic treatment of severe acne (Layton et al, 2006). However, isotretinoin can cause depression and an increased rate of birth defects if taken during pregnancy (Mondal et al, 2017). Thus, the drug is highly regulated by the US Food and Drug Administration.…”

“…Although topical retinoids (Vitamin A derivatives) have been on the market for about half a century, data regarding their use and/or exposure in pregnancy are limited. Nevertheless, malformations in the central nervous system (holoprosencephaly, (Wals et al, ), the cranium, the face, the heart and the thymus of infants of topical‐retinoid‐exposed pregnant women have been reported in individual case reports (Lammer et al, ; Mondal, S, & Mishra, ). Additionally, as mentioned before, RA has been shown to be critical for NC formation (Williams & Bohnsack, ).…”

“…Vitamin A embryopathy, a condition caused by in utero exposure to Vitamin A, is characterized by small deformed ears, mandibular hypoplasia, cleft palate and cardiac abnormalities. In effect, this disease has been shown to affect NC development (Mondal et al, ; Mulder et al, ). Additionally, treatment with Isotretinoin impaired normal craniofacial morphogenesis (Makori et al, ) and increased NCCs apoptosis via upregulation of the pro‐apoptotic transcription factor p53.…”
