Neurofunctional approaches play the paramount functions in management of neurological disorders to improve the functional capability after impairments and activity limitations. These interventional approaches aligned with the neuroplasticity theories and all rely on repetition matters to build up engrams for the change of the brain function and activity performance. Affolter approach guides cognitive perceptual interaction through tactile-kinesthetic inputs. Neuromuscular facilitation relays the periphery information to the central nervous system by joint and muscle stimulus by using different techniques such stretching, irradiation, traction and approximation. Neurodevelopmental therapy manages the abnormal movement and postures through hands on facilitation of normal movement and inhibiting abnormal patterns movement. Roods approaches focus on the primitive reflexes through sensory stimuli to the targeted sensory receptors to initiate the appropriate motor pattern development. Brunnstrom approaches build on the synergies to provoke the engagement of the affected limbs. Task-oriented approaches are based on motor learning and involve repeat training with task-oriented activities. It is effective for improvement of the functional performance. It is a training method for encouraging functional movement with an interesting task. And also it improved the dexterity when applied using mixed interventions in hemiplegic.

Introducción: la fractura de quinto metacarpiano es una patología que puede afectar funcionalmente a la mano. La reinserción a las actividades cotidianas o laborales se relaciona con el tratamiento recibido y el acceso a una rehabilitación temprana. Objetivo: comparar el tiempo de reinserción laboral, los resultados funcionales y clínicos de los pacientes con fracturas del cuello del quinto metacarpiano manejados con clavillos de Kirschner vía retrógrada versus anterógrada. Material y métodos: estudio comparativo, longitudinal, prospectivo en pacientes con fractura de cuello de quinto metacarpiano, con seguimiento clínico, radiográfico y con escala DASH (Disabilities of Arm, Shoulder and Arm) a la tercera, sexta y octava semana postoperatoria hasta su reinserción a sus actividades laborales. Resultados: se incluyó a 60 pacientes con fractura de quinto metacarpiano (58 hombres y dos mujeres), con un promedio de edad de 29.63 ± 10.15 años, tratados mediante reducción cerrada y estabilización con clavillo de Kirschner. El promedio de días necesarios para el retorno a las actividades laborales en el grupo de tratamiento mediante clavillo de Kirschner anterógrado fue de 27.35 ± 24.18 días, y el de la vía retrógrada de 66.56 ± 37.12 días (p < 0.001; IC 95% 14.4-63.9). Los pacientes con actividades laborales físicas ligeras requirieron 21.9 ± 25.11 días para reingresar a las labores por la vía anterógrada, comparados con los 63.7 ± 39.7 días de la vía retrógrada (p = 0.001; IC 95% 14. 93-68.58). Conclusión: el grupo de pacientes con fracturas de cuello del quinto metacarpiano manejados mediante clavillo de Kirschner anterógrado fueron capaces de realizar rehabilitación temprana y requirieron menos de la mitad del tiempo para regresar a sus labores, en comparación con el clavillo de Kirschner retrógrado.

Hand therapy: Inclusive care by South African physiotherapists

Hand therapy for individuals who sustained hand injuries is included in the Health Professions Council of South Africa (HPCSA) physiotherapy scope of practice. The training that physiotherapists receive at the undergraduate level lays the foundation for them to deliver hand therapy or hand rehabilitation according to the International Classification of Functioning, Disability and Health domains in a uniquely South African health service context. Further structured formal and informal postgraduate continued education opportunities may put physiotherapists in the ideal position in a multidisciplinary team to deliver optimal hand therapy. The problem is that the physiotherapist’s role in the multidisciplinary team delivering hand therapy in the past two decades has decreased, leaving room for a lack of health professional services in South Africa’s private sector where physiotherapists are often the first consultation in, for example, sports teams, but more pertinently, in the public and rural areas. The International Federation of Societies for Hand Therapy (IFSHT) practice profile and physiotherapy scope of practice, curriculum and education information assist in achieving the aim of this commentary to position physiotherapists in South Africa as primary health practitioners in delivering hand therapy.Clinical implication: The effective management of individuals with hand-related conditions and injuries is pertinent to ensure optimal hand function and quality of life. Equal continued formal education opportunities should thus be created for all multidisciplinary team professions at a postgraduate level.
