Reverse Dorsal Digital and Metacarpal Flaps: A Review of 27 Cases

Abstract: Reverse dorsal digital and metacarpal flaps use the dorsal skin of the digital or metacarpal areas, and they are based on the arterial branches anastomosing the volar and dorsal arterial networks of the fingers. These flaps are transposed as reverse island flaps. Dissection of the flap is easy, fast, and preserves the collateral nerve and artery to the fingertip. A series of 27 flaps is reviewed, with more than 6 months of follow-up. Skin defects in all patients were located over or beyond the proximal interph… Show more

“…Foucher & Braun's article (1979) was the first to describe the flap supplied by the first dorsal metacarpal artery (1DMA) 4 . In reviewing the literature regarding the anatomy of the dorsal arterial system of the hand, we noticed some controversy about the frequency of the dorsal metacarpal arteries (DMAs) [4][5][6][7][8] , the location of the distal cutaneous branches 5,[9][10][11][12] , and the amount of contribution of the dorsal and palmar arterial system to the formation of the DMAs [13][14][15] . Since that information is of extreme importance for the operative procedure to raise the flaps based on the DMAs, we found it to be useful to study the anatomy of the dorsal arterial system, focusing on the controversial aspects of the literature.…”

“…Pelissier and Casoli et al, 28 in their series of 27 patients, were the first who described pedicled island reverse flaps based on the dorsal branches of the propria digital artery to cover different digital defects. Previous well-conducted anatomical studies 3,5,12,23,26,30,33 provided the robust anatomical background for the design of such flaps.…”
