Reverse Superior Labial Artery Flap in Reconstruction of Nose and Medial Cheek Defects

Abstract: Reverse superior labial artery pedicled nasolabial island flap proved to be a good choice for reconstruction of the lower nose and moderate sized medial cheek defects. This pedicle should be considered where the defect is located on the course of lateral nasal artery or angular artery. Arc of rotation and reliability of this new flap is considered superior to angular artery and infraorbital artery-based nasolabial flaps where these arteries could also be used as pedicle.

“…This could be explained by the use of splited muscle pedicle which may works as tight filling of the tunnel plus care in this study towards meticulous closure at donor as well as recipient sites of the flap. Study proposes simple reconstruction for simple defect without any need for superior labial artery flap as in study conducted by Turan et al 15 This study describes split muscle pedicles neither epithelial turn-in, 16 nor extensive melolabial island flap 17 as they are not indicated in such situations.…”

“…Pedicle flaps mainly include the submental flap, 20,21 hemiforehead flap, 22 nasal flap, 23 retroangular flap, 24 orbicularis oculi myocutaneous flap in the temporal area, 25 and the recently reported reverse superior labial artery flap. 26 These flaps require dissection of the vascular pedicle, which is associated with timeconsuming surgery and scars left in the donor area. Free flaps require vascular anastomosis, and the color and texture of the flaps are different from the skin in the cheek.…”

“…[8][9][10] The greatest disadvantages of cheek rotation flaps are ectropion development and transfer of the hairy skin to the malar region. In case of nasolabial flaps, irregularities may develop at the rotation point.…”
