Revisiting a case of difficult airway with a rigid laryngoscope

Abstract: A cook airway exchange catheter is a long hollow and semirigid catheter which is routinely used for endotracheal tube (ETT) exchange in the intensive care unit setting or even in the operating room in an event of unexpected difficult airway. We describe a novel technique of using an ETT exchanger with the help of a rigid laryngoscope to facilitate intubation as a strategy in a patient with an anticipated difficult airway due to laryngeal cancer presenting as stridor with respiratory distress.

“…525 Case reports also observed intubation success for unanticipated and emergency airway patients when combination techniques were used (Category B4-B evidence). [526][527][528][529][530][531][532][533][534][535][536] The literature is insufficient to evaluate which of the above devices are most effective when attempted first after failed intubation, nor is the literature sufficient to evaluate the most effective order of devices to be used for attempted intubation of an unanticipated or emergency difficult airway.…”

“…Emergency extraction of FB under general anesthesia is the cornerstone of management to avoid further disastrous sequelae to the child. Anesthesiologists primarily use the Cook® airway exchange catheter (CAEC) to act as a conduit to deliver oxygen during endotracheal tube (ETT) exchange in the intensive care unit [3].…”
