Rheumatoid Factor and Disease Activity Are Independent Predictors of Lymphoma in Primary Sjögren's Syndrome

Abstract: Objective To define parameters predictive of lymphoma development in patients with primary Sjögren's syndrome (SS). Methods A multicenter case–control survey was performed to identify predictors of lymphoma. Cases were patients who developed lymphoma after diagnosis of primary SS and were mainly recruited through the Club Rhumatismes et Inflammation network. For each case, 2 controls (matched for disease duration and age) were randomly selected among patients with primary SS and without lymphoma. Cases and con… Show more

“…The mechanism behind this synergistic effect is unknown, but crosslinking and/or stabilization of immune complexes, consisting of autoantigens and autoantibodies, by RF is likely involved. In combination with the finding that higher RF levels seem to increase the risk of lymphoma [14], these data suggest that lowering RF levels by rituximab treatment is of clinical importance, as it may protect against disease progression and/or lymphoma development in pSS.…”

“…In pSS, circulating immunoglobulin M autoantibodies that are specific for immunoglobulin G-Fc, called rheumatoid factors (RFs), are found in the serum of 50%-60% of affected individuals. 16 In addition to previously known predictive factors of lymphoma occurrence, Nocturne and colleagues 17 recently demonstrated the independent role of RF in the development of this severe complication. In our series, out of seven patients, six tested positive for RF.…”

“…[5,40,41] Monoclonal mixed cryoprecipitates, reported as a detrimental prognostic factor for SS-related lymphomagenesis, [17] contain monoclonal RF, secreted by a subset of malignant B-cells derived by clonally expanded B cells exhibiting RF activity, [42] which has been emerged as an independent predictor for NHL in both Greek and French cohorts.
